- Scores must be reported by 9pm on the Tuesday after game day.
- Both teams must provide game scores.
- Score Discrepancies: The Home Team score will be used if the game scores do not match.
- Missing scores reported after 9pm on Tuesday must be emailed to the RCS Board. Please indicate Home/Visitor team status.
- Forfeits should be communicated to the RCS Board before game day or game time and cannot be reported via this form.
- Names for all Drop-In (DIP) and Pick-Up (PUP) and Free-Agent (FA) players, if any, must to be reported on this form.
- A photo of line-up card(s) must be uploaded. The line-up card for the home team (official book) must be signed by both coaches.
