Score Reporting Form

Scores must be reported by 5pm on the Tuesday after game day. The form will be unavailable after 5pm.
Both teams must enter the game score. Any discrepancies will be addressed by the RCS Board.
Missing scores reported after 5pm on Tuesday must be emailed to the RCS Board.
Forfeits should be communicated to the RCS Board before game day or game time and cannot be reported via this form.
Names for all Drop-In (DIP) and Pick-Up (PUP) and Free-Agent (FA) players, if any, must to be reported on this form.
A photo of line-up card(s) must be uploaded. The line-up card for the home team (official book) must be signed by both coaches.
